You will need to complete a client consent form (link) before your session and also a veterinary referral form (link) if you wish to claim from your insurance provider

Note: If you are making a claim through your insurance provider, you will need to pay for your session at time of booking and claim back through your provider using their claims process.  Please check before booking that our service is covered by your policy.
